Download the maintenance log
The workbook and CSV are editable. The PDF is designed for printing. All three use the same record fields and deliberately leave the service interval to the controlling local requirement.
Fields included
- Facility, device ID, location, manufacturer, and model.
- Local rule or permit reference.
- Inspection or service date, provider, and work performed.
- FOG or solids measurement only when locally authorized and safe.
- Volume removed when reported and receipt/manifest/disposal reference where required.
- Defects, repairs, corrective actions, next review date, manager initials, and notes.
Set the schedule correctly
First enter any local maximum interval, accumulation trigger, manufacturer instruction, or written adjustment. Then review the facility's own records after menu, volume, equipment, or operating changes. Do not turn a weekly check, monthly service, quarterly service, or 25% trigger from another jurisdiction into your rule.
Keep the original documents
This template organizes information; it does not replace an official manifest, invoice, service report, disposal receipt, permit form, or inspection notice. Keep the originals for the period required by the controlling rule.

Does this replace a manifest or service receipt?
No. Keep every original document required by the applicable program.
Should staff record a 25% measurement?
Only when the local rule permits that measurement basis and trained personnel can perform it without entry or unsafe exposure.
